THE FALSE PREMISE
This paper introduces a simple protocol to prevent cross infection in the clinical setting that appears to be overlooked in many non-theatre situations. Non-theatre clinical situations (one or no assistants) have been established and universally based on theatre protocol (two or more assistants) thereby leading to a false premise and false practice; to universally poor cross-infection control protocol (CICP).
The true premise is that the clear definition of the boundary between contaminated and non-contaminated, between scrubs assistant and scout assistant, is lost when one or no assistants are employed. Therefore to copy aseptic technique in the name of CICP, is false.
This paper defines the boundary, with the introduction of the concepts:
- The vector
- The chain of vectors
- CICP
- Appropriate barrier use and the avoidance of "back-contamination"
- Touch resonance
- The one-glove technique
- Efficient and effective surgery setup and take-down
- The weakest point and exponential chains of sepsis
- Some common breaches of protocol.
Cross Infection Control Protocol (CICP)
Aseptic Technique
- prevents all forms of contamination, including cross-contamination.
- Necessary in the operating theatre, not in the dental clinic.
- Requires both scrub and scout assistants.
CICP
- Breaks the chain of sepsis, preventing cross-contamination only
- Efficient with a single assistant -possible with no assistant
